The first time that the provider logs into the system, a box will appear confirming the use of an administrative proxy. The administrative proxy is someone that enters data on behalf of the provider. The provider will still need to review and attest.
The provider can assign a proxy by completing the requested information and signing the release of information which will appear by selecting Next. The assigned proxy will automatically be given access to the provider's ProApp and a notification is sent to the email provided. At this point, the provider can log out of the system to allow for the proxy to complete the form.
If the provider does not wish to assign an administrative proxy, clicking Cancel will remove the box. The box will still appear each time the provider logs in unless the Do Not Show Again is clicked.
A proxy may be added at any future date through the Proxies module in the menu on the left side of the screen.

- Personal email - Enter the practitioner's personal email address. **Note: this email address will be used to create the provider's Practitioner Center account, as well as where notifications will be sent regarding all credentialing items.
- Preferred method of contact - Select the best method to contact the provider from the drop down.
- Address - Enter the street address of the provider's primary office/practice location and select the appropriate complete address from the google list
- Telephone number - Enter the phone number for the practitioner's primary office location
- Fax number - Enter the fax number for the practitioner's primary office location.
- Office Representative First Name - Enter the first name of the practitioner's office manager or practice manager
- Office Representative Last Name - Enter the last name of the practitioner's office manager or practice manager
- Office Representative email - Enter the email address for the practitioner's office manager or practice manager
- Have you been known by or used any other name(s) - Select yes or no
- First Name - Enter the first name used in an Alias
- Middle name - Enter the middle name used in an Alias
- Last name - Enter the last name used in an Alias
- Suffix - Enter the suffix, if applicable from the drop down, used in an Alias

- The taxonomy code should automatically populate for the correct taxonomy.
- Is this your primary specialty - select yes or no. The practitioner must have at least one primary taxonomy. If the provider has more than one taxonomy, select the Add button to the right.
- Are you currently, or have you ever been board certified - select yes or no
- If yes, select the certifying entity from the drop down menu
- General certificate - select the certificate type from the drop down menu
- Subspecialty certificate - select the subspecialty certificate from the drop down menu, if applicable
- Certificate number - enter the practitioner's certificate number
- Is this your primary or secondary specialty - select primary or secondary
- Is this certification Active - select yes or no
- Are you participating in MOC - select yes or no
- Do you wish to be listed in teh directory under this specialty - select yes or no
- Is this certification lifetime or time-limited - select lifetime if "lifetime" is indicated on the certification. If the certificate has an expiration date, enter "time-limited".
- Date of original certification - enter the date the certification was originally issued
- If the certification is time-limited, enter the expiration date of the certificate
- Upload certification - It is required that a current copy of the certification be uploaded.
- If the provider has multiple certifications, then click the Add button on the right.

- Would you like to report a non-board certified specialty - select yes or no. If yes, enter the name of the specialty, describe this specialty, and upload a copy of the supported documentation
- The provider's information for Pre-Med or bachelor's degree should be entered in this section
- The provider's information for medical school or master's degree should be entered into this section.
The provider's internship, residency, and fellowship, if applicable, should be entered into this section. Non-physician providers will select No for this section.
- Select yes to open the necessary fields.
- Select the education type (internship, residency, or fellowship)
- Enter all required information and click Save
- After the information has been saved, click the Add button in the right column of the section. This will allow the additional information to be entered. Repeat steps ii and iii from above.
The provider's current employer should be entered into this section. This is the entity by which the provider receives income.
- If the provider has more than one employer, additional employers can be entered by clicking the Add button on the right of the screen after the first employer has been saved.
This section should be populated with all prior employer's for the provider
- If the provider has more than one employer, additional employers can be entered by clicking the Add button on the right of the screen after the first employer has been saved.
- The section must include all employers from the point of completing medical school or master's programs.
This section should contain all facilities where the provider currently has privileges to perform procedures and/or care for patients
- If the provider has privileges at more than one facility, additional facilities can be entered by clicking the Add button on the right of the screen after the first facility has been saved.
- The section must include all facilities where the provider currently cares for patients.
- If privileges are not required for the provider's type of care, select No for this section.
- For non-physician providers, all supervising physicians must be entered for each facility.
- For physicians who oversee providers (PA,NP, etc), those providers must be entered in the Sponsoring Practitioners area for each facility.
- For physicians, the individual or group that will serve as back up to the provider for call coverage must be entered.
This section should contain all facilities where the provider has had privileges to perform procedures and/or care for patients
- If the provider has had privileges at more than one facility, additional facilities can be entered by clicking the Add button on the right of the screen after the first facility has been saved.
- The section must include all facilities where the provider previously cared for patients.
- If privileges are not required for the provider's type of care, select No for this section.
Most providers will not populate this section. This section is if the practitioner provides care to patients at locations that are associated with the employer, such as a clinic.
- If the provider cares for patients at more than one clinic or location, additional entities can be entered by clicking the Add button on the right of the screen after the first entity has been saved.
- The section must include a contact for the location.
This section contains any employer where the practitioner receives income for work that is not for patient care, such as a business or administration.
- If the provider has more than one non-physician related employment, additional entities can be entered by clicking the Add button on the right of the screen after the first entity has been saved
If the provider has any postgraduate teaching or faculty appointments should be entered into this section. It must be an official position.
- If the provider has more than one non-physician related employment, additional entities can be entered by clicking the Add button on the right of the screen after the first entity has been saved
If the provider has served in any branch of the military, this information should be populated here.
- If the provider has served on more than one occasion or for more than one branch, additional information can be entered by clicking the Add button on the right of the screen after the first entity has been saved.
Military privileges are tracked somewhat differently from civilian facilities. If the provider has ever held privileges at a military facility, this information should be populated in this section.
- If the provider currently holds or has held privileges at more than one facility, additional facilities can be entered by clicking the add button on the right of the screen after the first facility has been saved.
It is part of the credentialing process that gaps of greater than 30 days be investigated. Enter any time frames where a provider was not employed as a practitioner
- If the provider had multiple instances, additional time frames can be added by clicking the add button on the right of the screen after the first facility has been saved.
The provider must enter at least three peer references in this section
- The peer must be of equal profession, ie. Physicians must have 3 physician, NP must have 3 NP's, etc.
- The peer must be associated with the provider for a period of longer than 6 months.
- The peer must also have working knowledge of the provider within the past 6 months.
- Additional peers can be added by clicking the add button on the right of the section after the first peer has been saved.
A copy of current and/or previous malpractice face sheets must be uploaded to this section, unless not applicable.
- Some provider types may be covered under a facility policy. For these providers, select Not Applicable for this section.
- For providers where the employer to which the provider is currently making application will be issuing the insurance and will be the only current coverage, the provider can select Not Applicable at this time. The insurance information will be added to the ProApp profile by the credentialing team.
- If the provider has had previous malpractice insurance coverage, this information must be listed in this section. Verification of prior malpractice claims is a credentialing requirement. The practitioner must provide the information for prior coverage in order to verify the claims history.
- Additional malpractice information can be added to this section by selecting the add button on the right side of the section.
- The provider may take advantage of utilizing the 3WON ProApp to maintain a complete provider profile (CV). This is not required information for the credentialing process.
- Information can be added by selecting a type under each section (Professional Organizations, Community service & charity, Achievements, Honors & awards, and Authorships & Presentations) and complete the requested information.
